APPOINTMENTS
INITIAL PSYCHIATRIC ASSESSMENT
All new patients must first complete an initial 90-minute psychiatric assessment. I will gather much information during this comprehensive interview. Please bring or email in any relevant helpful information such as records from other current or past providers. Before your first appointment, you will receive an email with a link to my online intake and consent forms. The fee for this visit is $450.
60-MINUTE COMPREHENSIVE FOLLOW UP
For comprehensive followup visits, 60-minute appointments are generally necessary. This allows me enough time to safely manage any prescribed medications and to adequately review updated collateral history from both you and your child's other care providers. The fee for this visit is $330.
30-MINUTE MEDICATION FOLLOW UP
For medication management visits, 30 minute appointments are generally necessary. This is usually sufficient time to safely manage all of your child's prescribed medications. The fee for this visit is $225.
OTHER CLINICAL SERVICES
For all other clinical services (consultations, writing reports, drafting letters, or completing forms) the fee is prorated to an hourly rate of $330/hour.
NURSING SERVICES
For emergency consultation with a nurse or home/office visits to receive injectable medication from a nurse, the fee is prorated to an hourly rate of $120/hour.
FORENSIC AND LEGAL SERVICES
For completion of forms requested by court or a lawyer, my fee is $450/hour. If testimony or deposition or an in-person court appearance is required, the fee is $550/hour.
CONCIERGE PSYCHIATRY SERVICES IN CONJUNCTION WITH JAY BERK PHD
We are proud to be able to offer a premium 120-minute comprehensive psychiatric assessment and concierge follow-up services in conjunction with Jay Berk PhD. Concierge fees are $600 for the initial assessment and $400 for follow up sessions.
Please contact Dr. Berk for a referral and to learn more about his concierge services, including psychological and psychoeducational testing, life coaching, and individual psychotherapy.
Medical Marijuana (MMJ)
I am certified to recommend medical marijuana in the state of Ohio. I conduct all MMJ appointments through Compassionate Cleveland. Appointments for new patients seeking an Ohio MMJ card assessment and existing patients seeking to renew their MMJ recommendations can be made on the Compassionate Cleveland website.
