Our client portal is where you go to manage your account, including updates to your contact or payment information. If any registration forms, consent forms, or release of information forms have been shared with you, this is where you go to complete and sign them online.

If for some reason you cannot access your registration forms using the client portal above, please click here to download a blank copy of our registration forms to complete, sign, and email back before your first appointment. If you are unable to complete the forms digitally as a PDF, please print them and send back scans/photos of the completed pages. Please make sure photos of forms are brightly lit and clear.

Please complete, sign, and email back a medical release form for each individual or group with whom you want us to collaborate. This may include your primary care provider, pediatrician, therapist/counselor, or family.
If someone in your family is helping to schedule and/or pay for your appointments, please complete a release form for this individual so that we may communicate with them regarding your account.
